Liability Release Form I understand that there are risks of physical injury associated with, arising out of, and inherent to the activities presented. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all rights and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue toSalted Dance Company LLC/ Salted Performing Arts/ Salted Community its officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “Salted Performing Arts or Salted Community ”. I hereby agree to release Salted Dance Company LLC / Salted Performing Arts/ Salted Community harmless of all liability and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation of the organization( the organization joining activities) on behalf of the all the participants. I am aware that this is a release of liability and an acknowledgment of the organization's voluntary and knowing assumption of the risk of injury and death. I have signed this document voluntarily and of my own free will in exchange for the privilege of participating in the organization, I am representing my organization and enrolling in the membership program. I also give Salted Performing Arts and Salted Community permission to use pictures of my childcare home and center in or on any form of advertisement for Salted Dance Company LLC/ Salted Performing Arts and Salted Community or a Salted Performing Arts Salted COmmunity affiliated event. The participant/organization has my permission to participate in Salted Dance Company LLC/ Salted Performing Arts/ Salted Community Events. I warrant the below information is complete and correct. I further release Salted Dance Company LLC/ Salted Performing Arts/ Salted Community of all liabilities associated with my child’s attendance at SSalted Dance Company LLC/ Salted Performing Arts/ Salted Community . *Tuition, Subscription payments, enrollment fee, and costumes fee are nonrefundable* Salted Dance Company LLC/ Salted Performing Arts/ Salted CommunityArts reserves the right to suspend or expel its joined organizations as it deems fit.
Membership:
I agree that membership fees are due monthly regardless of whether my facility participates that week or not. Membership fees cover only the membership monthly fee. All other fees are additional charges. Membership Fees are not prorated for missed classes, inclement weather, holidays, and any disaster.
Childcare Facilities and Childcare homes are responsible for paying the monthly fee to Salted Dance Company LLC/ Salted Performing Arts/ Salted Community.
Cancelation:
To cancel membership after the commit months have been accomplished the notice must be given 30days before the next billing cycle via email must be sent to Salted Performing Arts(SPA).
Term Contract:
Each Facility commits to a minimum 4 month with our organization.
Organization compliance agreement:
Each organization and its participants( including organizations: students, staff, parents, and family members of students and staff must follow the guidelines and policies of Salted Dance Company LLC/ Salted Performing Arts/ Salted Community. Failure to do so could lead to membership cancelation and denial of further participation with SPA.
SPA Cancelation:
In case of class cancelation on the part of Salted Dance Company LLC/ Salted Performing Arts/ Salted Community will do its best to reschedule the canceled class if it is within its means to do so.
Seasonal Concerts and Events :
All membership holders are eligible to participate in seasonal dance concerts and events. It is the responsibility of the organization to coordinate information and concert fees for its clients.
Cancellation of Subscription. In the event that the Customer would like to cancel their subscription they must inform Salted Community of their intention to cancel their subscription at least seven (30 ) days before the next scheduled payment in their billing cycle to avoid being charged for the following month via email. Your subscription can be cancelled after the 4 month period, if not cnaceld by the end of the four month period Customer will be automatically enrolled into the next four month period.
